Lower Trapezius Tendon Transfer

 

Lower trapezius tendon transfer is a new procedure for massive or irreparable rotator cuff repairs. 

 

Rotator cuff tears are one of the main causes of shoulder pain and rotator cuff repair is often an excellent solution. However, this surgery is not always suitable when rotator cuff tears are large or following re-rupture of a repaired tendon. One option is to have a reverse shoulder replacement.  But to preserve the joint and maintain a high level of activity, lower trapezius tendon transfer may be an option. 

 


Surgery

In lower trapezius tendon transfer, a small portion of the trapezius muscle (on your back) is transferred with the use of a tendon graft to take over and do the work of the rotator cuff muscle which is not working.

 

Surgery takes between 1 – 2 hours. Most of the surgery is performed arthroscopically (keyhole surgery) but incisions are required to transfer the lower trapezius.

 


After Surgery

The post-operative rehabilitation is very similar to normal rehabilitation after a rotator cuff repair. You will be closely monitored after surgery and you will usually be able to go home the same or next day.

 

The shoulder is immobilized in a brace, keeping the arm externally rotated, for about 6 weeks. Internal rotation of the arm is avoided during this time. After 4-6 weeks from surgery, physical therapy will begin.

 

Full healing typically takes around 3 months and improvement of motion and strength build over one year.
