• A person has high blood pressure or has experienced any symptoms 
• Suffers from high cholesterol

• Lived a lifestyle that has involved – smoking or excessive consumption of alcohol

• Initial appointment with a cardiologist
• Diagnostic blood tests to check cholesterol and glucose levels 
• Blood pressure reading
• ECG – (recording the electrical activity of the heart over a short period of time)
• Telephone follow-up 

*Face-to-face follow-up appointments will be charged at an additional cost
