• Based on a face-to-face consultation

• Issued by a qualified GP

• Written in clear, formal wording suitable for official submission

• Provided as a printed and/or digital copy

Certificates can only be issued where there is appropriate medical evidence. In some cases, additional tests or documentation may be required before a certificate can be completed.

To help our clinicians complete your certificate without delay, please bring:

• Photo identification

• Relevant medical records or test results

• Details of the requesting organisation (employer, insurer, school, etc.)

• Any forms or templates you have been asked to provide

If the certificate relates to a previous illness or absence, supporting dates and timelines will also be helpful.

The fees and turnaround times vary depending on:

• The type and complexity of the certificate

• Whether an examination or report review is required

• The level of clinical detail requested
